**Excess length of stay** in SUS (Sistema Único de Saúde, Brazil's public health system) is the set of days patients admitted through SUS stay in hospital beyond the expected stay for their procedure. Since the payment for the admission is, as a rule, defined by the procedure on the AIH and not by the number of days, every day beyond the expected stay is cost for the hospital without matching revenue.

## How SUS pays for admissions

For a SUS admission, the hospital issues an AIH (Autorização de Internação Hospitalar, the hospital admission authorization) with a main procedure. Each procedure has, in the SUS procedure table (SIGTAP), a value and an expected **average length of stay**. The AIH value does not automatically increase if the patient stays longer.

There are specific rules for long admissions. For procedures that allow an **extended stay** (an attribute of the procedure in the table), the SUS Hospital Information System (SIH/SUS) manual provides for recording extended-stay daily rates when the admission exceeds twice the procedure's average length of stay, excluding ICU days. The full rules and exceptions are in the SIH/SUS technical manuals and should be checked by the hospital's SUS billing team.

## Why excess days matter

- **Cost without revenue**: every day beyond the expected stay consumes the daily rate, staff, supplies and drugs.
- **Capacity**: an occupied bed cannot take a new patient, which weighs on hospitals with waiting lists.
- **SUS result**: excess days are one of the main reasons SUS margin is lower than expected.

## How to measure

- **Excess days**: for each admission, the days beyond the procedure's expected stay.
- **Financial impact**: excess days multiplied by the department's average daily cost.
- **Concentration**: by procedure, specialty, department and reason for delayed discharge.
- **Trend**: month by month, to see whether actions are working.

The Clic Health SUS Excess Stay dashboard shows procedures that exceeded the expected stay and the monthly financial impact.

## Most common causes

- Delays in tests, specialist opinions or procedures during the admission.
- Clinical complications and healthcare-associated infections.
- Patients who could already be discharged but depend on a place in another service, home care or social issues.
- No discharge planning from admission.

## How to reduce it

The practices are the same as for managing [average length of stay](https://clichealth.com.br/en/blog/average-length-of-stay), applied with priority to SUS admissions:

- a daily list of SUS patients above the expected stay, with the reason;
- faster tests and specialist opinions for those patients;
- coordination with the care network for transfers and transitional care;
- recording reasons for delays, to tackle the recurring ones.

## Related questions

Are excess stay and extended stay the same thing?Not exactly. In management, excess stay usually refers to all days beyond the expected stay. Extended stay is the SUS billing rule for recording daily rates on procedures that allow it, from twice the average length of stay.

Does every procedure allow an extended stay?No. It is an attribute defined for each procedure in the SUS table. The hospital's SUS billing team should check each procedure's attribute.

Do excess days appear in the income statement?Indirectly: as daily costs without equivalent revenue. That is why it helps to track excess days alongside the SUS income statement.
